How to Qualify: RN Care Manager: Education Requirements: ● Graduation from a state-accredited school of professional nursing. ● If hired after October 1, 2015 must be enrolled in an accredited program within four years of employment, and obtain a Bachelor's degree with a major in Nursing or a Master's degree with a major in Nursing within seven years of employment date. Licensure/Certification Requirements: ● Licensed to practice as a Registered Nurse in the state of North Carolina. Professional Experience Requirements: ● Two (2) years of health care experience as a Registered Nurse. Knowledge/Skills/and Abilities Requirements: ● Strong assessment and critical thinking skills Utilization Manager: Education Requirements: ● Graduation from a state-accredited school of professional nursing or other professional degree with appropriate clinical and/or project management experience. ● If hired after October 1, 2015, must be enrolled in an accredited program within four years of employment, and obtain a Bachelor's degree with a major in Nursing or a Master's degree with a major in Nursing within seven years of employment date. Licensure/Certification Requirements: ● Licensed to practice as a Registered Nurse in the state of North Carolina. Professional Experience Requirements: ● Two (2) years of clinical experience in a medical facility and/or comparable Utilization Management experience. Care Manager (MSW): Education Requirements: ● Requires Master's in Social Work. Licensure/Certification Requirements: ● LCSW required. Professional Experience Requirements: ● Three (3) years of broad clinical experience in the acute care setting caring for patients with complex medical conditions required. Experience in case management, utilization review, quality management preferred. Knowledge/Skills/and Abilities Requirements: ● Excellent interpersonal skills and communication capabilities. Knowledge of care management and quality improvement principles. Possess appropriate analytical skills to independently problem solve and make decisions. Possess computer skills at a level sufficient to effectively and efficiently use computer applications common to a Patient Centered Medical Home practice. Clinical Dietitian: Education Requirements: ● Bachelor’s degree at a US regionally accredited university or college and course work accredited or approved by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) of the Academy of Nutrition and Dietetics ● Completion of a Coordinated Program in Dietetics or Dietetic Internship accredited by ACEND (Accreditation Council for Education in Nutrition and Dietetics) of no less than 900 hours (1200 hours for graduates of programs beginning 2012) Licensure/Certification Requirements: ● Registered Dietitian (RD) by The Commission of Dietetic Registration (CDR) ● Licensed as a Dietitian/Nutritionist (LDN) by the North Carolina Board of Dietetics and Nutrition. (Certain positions may allow licensure to be obtained within one (1) month from date of hire). Professional Experience Requirements: ● Prior experience may be waived for specific positions. Additional experience may be required for specific positions. UNCMC Cardiac Rehab: Two(2) years as a clinical dietitian in a cardiac rehabilitation program or outpatient cardiology clinic. Knowledge/Skills/and Abilities Requirements: ● • Knowledge of the Nutrition Care Process and ability to apply all steps of the process. • Knowledge and application of medical nutrition therapy. • Understands and can implement Motivational Interviewing • Critical thinking to integrate facts, informed opinion, active listening, and observations. • Decision making, problem solving, and collaboration. • Ability to communicate effectively in both written and verbal form to patients, public, hospital and medical staff, and physicians. • Ability to function independently on assigned patient care units. • Ability to counsel and educate others. • General knowledge of nutrient analysis, word processing, and spreadsheet software.
